By submitting this application request electronically to the Credit Union, I/we authorize the Credit Union to check my/our Chexsystems history and my/our credit history for the purposes of opening an account. By completing this application, I/we represent and warrant that I/we have the financial ability and an intent to repay this loan. I/We am/are currently receiving sufficient income to repay this loan and certify that my/our credit rating is in good standing. There are no adverse facts which concern my/our finances to disclose to the Credit Union which may affect my/our ability to repay this loan. I/We authorize any person, association, firm or corporation to furnish on request of this Credit Union information concerning my/our affairs. (Section 1014, Title 18, U.S. Code, makes it a Federal crime to knowingly make a false statement or report in the application for the purpose of influencing the Credit Union.) Upon approval of this application, I/we agree to be bound by the terms and conditions of the Visa and ATM Card Agreement and Disclosure which will be sent to me/us when my/our card(s) is/are issued.
